NEW LAB DIRECTOR / CONSULTANCY START-UP

•  Incorporate an available company name in your selected state
• Generate your Articles of Incorporation
• File for your Federal Tax ID number
• File your S-Corp Election (optional, but recommended for tax planning purposes)

 Provide:

• A template of a Client Engagement Contract (.docx) that you can modify as necessary for all future clients (This alone costs over $2000 to have an attorney generate)
• A PDF copy of your state Articles of Incorporation
• Your Federal Tax ID number with accompanying IRS letter
• Instructions for opening your business bank account
• A list of basic tips to keep small business ownership painless

We will be available for consultation should questions arise in regard to the above. 

 1. Satisfy the Qualifications to become a CLIA lab director*; available here:  https://www.ecfr.gov/current/title-42/section-493.1443 

*Not required unless you will direct laboratories as part of your business.

2. Provide the specifics of your business here and make your initial payment of $500.

3. Receive a copy of the application we submitted on your behalf to the state for your company’s articles of incorporation, along with the link to make your final payment of the state's filing fee + $1500. 

4. When your final payment is received, you will receive the following doocumentation by email, in a .zip file:

• A template of a Client Engagement Contract (.docx) that you can modify as necessary for all future clients
• A PDF copy of your state Articles of Incorporation
• A PDF copy of Certificate of Incorporation
• Your Federal Tax ID number with accompanying IRS letter
• Instructions for opening your business bank account
• A list of basic tips to keep small business ownership painless

• We offer live, remote, one-on-one QuickBooks training for an additional fee of $100. This is a 1-hour introductory lesson to help you set up an accounting ledger, understand how to log income and expenses, as well as generate tax reports for your accoutant. 
• Additional training packages available upon request at a negotiated rate. 
• Lab Director Placement services may be offered for an additional fee, subject to availability and individual qualifications.
